Registration Form, Procedures, and Policies

Please note: The registration form requests your social security number (SSN).  The SSN you provide for enrollment purposes, or when requesting specific services, will be used by the University to verify your identity for official record keeping and reporting.  If you choose not to supply your SSN, certain services, such as transcripts, enrollment verification, tax reporting, financial aid and other services may not be available to you, and Penn State cannot guarantee a complete academic record for you.  Your SSN will be stored in a central system and only used for official reporting and record keeping.  It will not be used as a primary source to identify you within the Penn State system; the PSU ID will be used as the primary identifier.

Registration Procedures

By Mail:  Complete the registration form and send it to Penn State Erie, Continuing Education, Logan Carriage House, 4213 Station Road, Erie, PA 16563-1000.  Fees may be paid by check, money order, VISA, MasterCard, Discover, or request to bill employer.  Employer billing requests must be accompanied by a letter of authorization.  Checks should be payable to The Pennsylvania State University.

By Phone: The phone number is (814) 898-6103 (Eastern Time).  Please call 24 hours a day - 7 days a week with your registration information and your phone number.

In Person:  Please stop in the Continuing Education office located at the Logan House property on Station Road, Monday through Friday, 8:00 a.m.-5:00 p.m., or appointments can be made Monday through Thursday, 5:00-9:00 p.m. if necessary.

By FAX:  Students may register by FAX if paying by credit card or by company billing.   A letter of authorization on company letterhead or company P.O. must accompany billing requests.  The FAX number is (814) 898-6053.

Cancellations

The University may cancel or postpone any course or activity because of insufficient enrollment or other unforeseen circumstances.  If a program is canceled or postponed, the University will refund the registration fee but cannot be held responsible for any other related costs, charges, or expenses, including cancellation/change charges assessed by airlines or travel agencies.

Refunds

Refunds will be made for cancellations received eight business days before a workshop's start date.  Anyone who is registered but cannot attend may send a substitute to that particular workshop.
